According to South Korea's National Intelligence Service (NIS), his weight gain and heavy smoking habits increase the risk of heart and blood vessel diseases. A South Korean deputy, Youn Kun-young, disclosed this information based on explanations provided by the NIS. He mentioned that Kim Jong-un's obesity and smoking habits could escalate health complications. However, the NIS stated that there are no indications of significant health problems affecting Kim's ability to perform his duties. Youn noted that Kim demonstrates the ability to engage in physical exercises and even jogging.

Kim Jong-un's health has been a subject of concern for several years. In 2020, he was absent from public appearances for nearly three weeks, sparking speculation about his health. His absence coincided with the anniversary of his grandfather, Kim Il Sung's, birthday. There were rumors that he might be seriously ill or incapacitated. Kim's father, Kim Jong Il, and grandfather, Kim Il Sung, also struggled with obesity and smoking, ultimately dying from heart-related issues.

These health concerns arise as North Korea faces significant food shortages. The United Nations frequently reports that food scarcity and poor nutrition are major humanitarian challenges in the country. Despite these challenges, Kim Jong-un is known to have a preference for expensive and exclusive foods. Reports suggest that he enjoys caviar, lobster, foie gras, and high-end meat.

According to Paul Smart, a former chef at the Metropole hotel in Hanoi, where Kim Jong-un met with US President Donald Trump in 2019, Kim has a particular fondness for expensive delicacies. Smart mentioned that Kim's favorite dishes include Wagyu beef from Japan, which is renowned for its high quality and price. Kim reportedly prefers his meat cooked to a specific doneness.

Kim Jong-un's health could have implications for North Korea's leadership, given the country's possession of nuclear weapons. In recent months, the NIS has suggested that Kim's daughter, Kim Ju-ae, may be being groomed as a potential successor. Kim Ju-ae, believed to be around 13 years old, has increasingly appeared alongside her father at significant events, including military exercises and industrial launches.

The NIS is also investigating whether Kim Jong-un has a son who could potentially succeed him.
